When visiting our office, it is helpful for all of our patients, new and established, to bring with them a list of any medications they are currently taking.  In addition, the following documents are available for you to download on your computer and print at home at your convenience.  Just simply click on a link and complete the forms you need.  If you are a first time patient, please select 'Atlantic Eye Consultants - Patient Form Packet' and complete all of the forms prior to your appointment.  Fill out the information to the best of your abilities and if you have any questions, our staff will gladly assist you when you arrive for your visit.
